Player Registration Instructions

Registration is online only, through a parent account on this site.

Payment must be submitted at the time of registration, except for tryout and waitlist registrations. Visa/Mastercard accepted. Email the AYSO418 Registrar at [email protected], for instructions to pay by check or to make arrangements to register in person.

Player registrations must be completed by the player's legal parent or guardian.

Have These Items Ready

To make the player registration process quick and easy, have the following items on hand:

Proof of Age (New Players Only) - a clear scan or photo of birth certificate or passport

Physician Name & Phone Number

Medical Insurance Provider Info

Emergency Contact Info

Concussion Information Sheet - Review the information in this sheet with your child, and keep a copy for your reference. You will be required to sign the Concussion Information Sheet during the player registration process.

AYSO Player General Release Form - Submit this form to the coach if your child will be walking, biking, or driving themselves from practices or games.

AYSO Kids Zone Parent Pledge - The AYSO soccer fields are a safe, friendly, happy, place for children to play and experience the joy of soccer without fear of judgement. We want all players, coaches, families and volunteers to have fun! All participants, volunteers, and spectators are encouraged to use positive language, demonstrate good sportsmanship, and create a great experience and positive environment for every player. Parents and spectators are asked to adhere to the expectations and guidelines set forth in our Parent Pledge. Review the Parent Pledge and keep a copy for your reference.
